- The original copies of the “Application Form for Self-assignment of Places in Secondary One” issued by the Education Bureau (including the “Education Bureau copy”, the “School copy” and the “Parent copy”)
- Application Form for Self-allocated Degrees in Form 1 of Secondary School
- Original identity document
- Copies of Xiao Wu’s report cards for the second semester of the fifth grade and the first semester of the sixth grade, as well as certificates documenting his achievements in various activities and services
- Catholics are requested to attach a copy of their baptismal certificate (if applicable).
- Two return envelopes have been prepared, each containing a $2.2 postage stamp and filled in with the student’s address.
