- The three interconnected sections of the “Application Form for Self-assignment of Places in Grade 7” issued by the Education Bureau – namely the “Education Bureau copy”, the “School copy” and the “Parent copy” – must not be torn apart.
- Copies of the report cards for the second semester of Grade 5 and the first semester of Grade 6 (if the primary school fails to distribute the Grade 6 report cards during the registration period, the applicant must submit them before the interview date).
- Submit in person or entrust a relative or friend to do so: Present the original identification documents of the applicant (such as ID card or passport).
- Two return envelopes already stamped with postage (HK$2.2 each). Please write the address in block letters on the envelopes. The recipient must be the applicant student.
