Required documents

• Identity documents of the applicant (parent/guardian) and their copies
• Birth certificate and its copies of the applicant student
• If the applicant’s sibling is currently enrolled at this institution: The sibling’s birth certificate and its copy; a copy of the “Student Handbook” (page 3) and the “Registration Status Inquiry Form” of the sibling (the photograph on these documents must be stamped by the school).
• If the applicant’s parent/sibling is a graduate of our school (Ciyunshan Catholic Primary School/Huang Daxian Catholic Primary School (Morning Session)): The graduate certificate and its copy of the parent/sibling; The birth certificate and its copy of the parent/sibling.
• Proof documents and copies of the residential address declared (such as utility bills, lease agreements with stamps affixed, public housing lease contracts, or water, electricity, gas, or residential telephone bills from the past 3 months). The name on these documents must match the name of the parent/guardian.
• If the applicant is a Catholic believer: The baptism certificate of the applicant and its copies
• Prepare a return envelope stamped with a $2.2 postage fee, and write on it the student’s Chinese name and address for future communication purposes.

Selection criteria

1. Of appropriate age
2. My elder brother/sister is studying at this school.
3. The father/mother/brother/sister is a graduate of this school (Ciyunshan Catholic Primary School/Huang Daxian Catholic Primary School Morning Session).
