- Copy of the academic report card for the senior class in kindergarten
- Copies of materials related to sports and cultural activities, competition awards, and services (please print on both sides; a maximum of 5 sheets are allowed).
- If the parent is a graduate of our school, a copy of the graduation certificate must be attached.

Application forms can be submitted from Wednesday, June 3, 2026, to Tuesday, June 9, 2026. Hours of submission are as follows: Monday to Friday: 8:30 a.m. – 5:00 p.m.; Saturday: 8:30 a.m. – 1:00 p.m. Applications can be submitted in person at the school, via express delivery, or by mail. Applications submitted after the deadline will not be accepted.
