Required documents

• Student birth certificate/identity document
• Identity certificate of the applicant (guardian)
• Proof of address (any one of the following: a leased contract with stamps affixed, an HOS rental card, a food subsidy receipt, a water/electricity/gas/residential fixed-line telephone bill, etc.)
• If the parent or sibling of the applicant is a graduate of this school, copies of the graduation certificate and identity document must be attached.
• If an applicant has a sibling studying at this institution, a photocopy of the cover and the third page of the sibling’s student ID booklet, as well as a photocopy of their identity document, must be attached.
• On the completed application form for primary one admission, in the section where you need to specify the name of the primary school you wish to apply to, you must fill in “Le Shan Tang Leung Wong Wai Fong Memorial School”.
• The original and photocopies of the birth certificate of the applicant (if the applicant was not born in Hong Kong, a copy of the birth certificate issued in another place and a copy of the identity document proving residence in Hong Kong must also be provided).
• The original and photocopies of the residence address verification documents submitted (such as a leased contract with stamps, an HOS rental card, a food subsidy receipt, or any one of the bills for water/electricity/gas/residential fixed-line telephone services). The name on this document must match the name of the parent/guardian.
• Original ID card of the parent/guardian (if the parent/guardian entrusts someone else to submit the application form on their behalf, this person must present a photocopy of the parent/guardian’s ID card).
• If you have a brother or sister studying at this school, please attach copies of their birth certificate and the pages of their student record book that contain relevant information about their enrollment status.
• If you declare that your father/mother or elder sibling is a graduate of this school, please attach the birth certificate of your elder sibling, as well as the original and photocopies of their graduation certificate.
• Registration form (including student photo)
• Copy of the registration certificate for the first grade
• Apply for a copy of the school selection form for the unified placement system
• Report card or evaluation form for kindergarten students
• If you choose our university during the “self-assignment of places” phase but are not assigned to study here, please bring a copy of the “Self-Assignment of Places Application Form” (yellow).
• Applicants may submit a “self-introduction document” consisting of no more than 4 A4 pages (which does not need to be enclosed in a folder).
• Other attachments, such as recommendation letters.

2. The parents are graduates of this school.
3. My elder siblings are both graduates of this school.
4. My elder brother and sister are both studying at this school.
5. I applied to this university during the period when degrees were allocated on a self-designated basis.
6. Choose our school as your first choice during the unified placement process (Category A or Category B).

Selection weightings

The scores are calculated in accordance with the “Scoring Method Guidelines” set by the Education Bureau.
