Required documents

• The original and photocopy of the child’s Hong Kong birth certificate, or the original and photocopy of the Hong Kong residence permit.
• Original identification documents of the parent/guardian. <To be returned after verification>.
• The original and photocopies of the residence address verification documents submitted by the applicant (such as water/electricity/gas bills, lease agreements with official stamps, food delivery receipts, residential telephone bills, etc.) must bear the same name as that of the parent/guardian.
• If the applicant declares that their elder brother/sister is enrolled in this school, they must submit a photocopy of the elder brother/sister’s student handbook (“Registration Record”) and present both the original and a photocopy of the elder brother/sister’s birth certificate.
• If the applicant declares that one of their parents or siblings is a graduate of this school, they must present both the original and a photocopy of the respective graduation certificate. If the applicant declares that a sibling is a graduate of this school, they must also present both the original and a photocopy of the sibling’s birth certificate.
• If parents/guardians entrust an authorized person to submit the form on their behalf, they must submit both a copy of their identity documents and the authorization letter together with the form.
