Required documents

• The application form, which has been completed and signed by the parent or guardian of the applicant (the child who intends to enroll in our school), in triplicate.
• Original and duplicate copies of the applicant’s Hong Kong birth certificate (if the applicant was not born in Hong Kong, please provide the original and duplicate copies of the applicant’s birth certificate from another place, as well as the identity documents proving that the applicant has been granted permission to reside and study in Hong Kong).
• Original and copy of the ID card or identity verification document of the applicant’s parent or guardian (if the parent or guardian is unable to submit the application in person, the person attending must present a power of attorney issued by the applicant’s parent, as well as a copy of the parent or guardian’s identity verification document).
• Original and duplicate copies of the documents proving the residential address stated in the application (the name on the documents must match the name of the applicant’s parent or guardian). Acceptable documents include water/electricity/gas/fixed-line home telephone bills, food delivery receipts, public housing rental cards, or rental contracts that have been stamped.
• If you choose to submit documentation stating that your elder brother or sister is enrolled at this school under Option (C, Item [A]), the following documents must be provided: a. Both original and duplicate copies of the birth certificate of your elder brother or sister; b. A duplicate copy of the “Student Record Sheet” (pages 2–3) from your elder brother or sister’s student handbook (must be accompanied by the signature of the parent).
• If you choose Option (Part C [Item 4]) and declare that your father/mother/elder brother/elder sister is a graduate of this school, you must present the following documents: a. The original and duplicate copies of your father/mother’s graduation certificate; or b. The original and duplicate copies of your elder brother/elder sister’s graduation certificate, as well as their original and duplicate birth certificates.
• If you choose to declare your religious belief under Option (Part C [Item 6]), the following documents must be presented: a. The original and duplicate copies of the baptism certificate of your father/mother; b. The original and duplicate copies of the applicant’s baptism certificate.
• If the applicant has not yet been baptized, it is necessary to provide a baptism certificate of the parent as well as a letter of certification issued by the pastor of the church to which they belong, in order to confirm that the applicant child regularly attends church activities such as worship services or Sunday school classes.
