Required documents

• The completed and signed application form for “self-assignment of a degree”
• Original and duplicate copies of the child’s Hong Kong birth certificate (if the child does not possess a Hong Kong birth certificate, please provide the original and duplicate copies of the non-Hong Kong birth certificate and the identity document approving his/her residence in Hong Kong).
• Original identification documents of the parent or guardian
• Original and copy of the address verification document (it must be the public housing lease agreement, the stamped lease agreement, or a document issued by a government agency of the parent or guardian. Examples include water/electricity/coal bills, residential fixed-line telephone bills, or food subsidies receipts. If the name on the address verification document does not match that of the parent or guardian, a relevant birth certificate or identity document must be provided to prove the relationship with the student).
• If applying for Category C (Options A or B), please submit the following documents: A. If the sibling is enrolled in this school: ~ Original and copy of the sibling’s birth certificate; ~ Original and copy of the sibling’s current student handbook. B. If the parent/sibling is a graduate of this school (including graduates from St. John’s Chao Cheung Primary School (Morning Session) or St. John’s Li Chao Cheung Primary School (Morning Session)): ~ Original and copy of the parent/sibling’s graduation certificate; ~ Original and copy of the sibling’s birth certificate. C. If the parent/sibling belongs to the same religious denomination as the student, and has been baptized in a Christian or Catholic church: ~ Original and copy of the baptism certificate or proof of registration as a member of the church. D. If the applicant is a member of our organization, i.e., an employee of St. John’s Primary School (Hong Kong) Management Committee Limited: ~ Original and copy of the employment certificate.
• If entrusting someone else to submit the form on your behalf, you must present both the original and copies of the power of attorney as well as the identity documents of the person authorized to act.

2. The parent/sibling is a graduate of this school (including graduates from St. John’s Chao Cheung Primary School (Morning Session) and St. John’s Li Chao Cheung Primary School (Morning Session)).
3. The same religion
4. Members of this association
5. Apply to our university under the "Self-allocated Degree" program.
6. When filling out the “Unified Allocation Form,” please indicate St. John’s Li Zhaoqiang Primary School as your first choice.
