• The completed and signed application form for “self-assignment of a degree”
• Original and duplicate copies of the Hong Kong birth certificate for the child applying (if the child does not have a Hong Kong birth certificate, please provide the original and duplicate copies of the non-Hong Kong birth certificate and the identity document approving his/her residence in Hong Kong).
• Original identification documents of the parent or guardian
• Original and duplicate copies of the address verification document (it must be the public housing lease agreement, the stamped lease agreement, or a document issued by a government agency of the parent or guardian. Examples include water/electricity/coal bills, residential fixed-line telephone bills, or food subsidies receipts. If the name on the address verification document does not correspond to that of the parent or guardian, a relevant birth certificate or identity document must be provided to prove the relationship with the student).
• If applying for Category C (Items A and B), please submit the following documents: If the sibling is currently enrolled at this school: - The original and duplicate of the sibling’s birth certificate; - The original and duplicate of the page containing the sibling’s student information in this academic year’s student handbook. If the parent/sibling is a graduate of this school (including graduates from St. John’s Chao Cheung Primary School (Morning Session) or St. John’s Li Chao Cheung Primary School (Morning Session)): - The original and duplicate of the parent/sibling’s graduation certificate; - The original and duplicate of the sibling’s birth certificate. In cases where the parent/sibling and the student belong to the same religious denomination: - The original and duplicate of the baptism certificate or proof of registration as a member of a Christian or Catholic church. If the applicant is a member of an association affiliated with this school: - The original and duplicate of the employment certificate issued by The Anglican Primary School Supervisory Committee Limited.
• If entrusting someone else to submit the form on your behalf, you must provide both the original and copies of the power of attorney as well as the identity documents of the person authorized to act.
