- Application for the original and duplicate copies of the birth certificate of a child (if the child was not born in Hong Kong, the birth certificate from the place of birth and the original and duplicate copies of the documents proving residence in Hong Kong must also be submitted).
- Proof of Residence [Original and photocopies of the documents proving your place of residence (such as a leased contract with stamps, a food delivery receipt, an HOS rental card, or bills for water/electricity/gas/residential telephone). The name on these documents must match the name of the parent/guardian.]
- If applying on behalf of a child or their parent who is a member of the Christian or Catholic Church, both original and duplicate copies of the “Church Member Baptism Certificate” must be submitted.
- If your brother/sister is studying at this school, you must submit a copy of the first page of their semester handbook, as well as the original and duplicate copies of their birth certificate.
- If the father/mother/elder brother/sister is a graduate of this school, it is necessary to submit both original and duplicate copies of the elder brother/sister’s birth certificate, as well as both original and duplicate copies of the father/mother/elder brother/sister’s graduation certificate.
- Parent identification document (if a parent entrusts someone else to submit the form on their behalf, a copy of the parent’s identification document must be provided).
- If you submit a paper application through our school, you must bring a copy for the parent(s) along when registering (the yellow form).
- Fill out the application form and attach a recent photo (id card photo).
- Present the original birth certificate of the student and submit a copy: a. For children born in Hong Kong: Hong Kong Birth Certificate; b. For children born outside Hong Kong: Travel Permit, Return Permit, Visa Identity Card or Passport.
- Present the student’s most recent academic report card, certificates of award, and submit copies of these documents.
