- The original version of the “Application Form for Self-Allocation of Places in Secondary One” printed by the Education Bureau
- Copies of Xiao Wu’s academic report cards for the first and second semesters
- A copy of Xiao Liu’s report card for the first semester of junior high school (if it has not been distributed, it can be sent back later).
- Copies of certification documents for services/activities/talents
- A copy of the applicant’s ID card
- Also provide an return envelope with sufficient postage already affixed.
