Required documents

• Submit the original and duplicate copies of the ID card of the parent/guardian filling out the form.
• Apply for the original and duplicate copies of the birth certificate for the child
• Water/electricity/gas/ration/home telephone/home broadband/lease agreement (proof of address for parents/guardians) – original and copies, or printed online receipts (the receipts must be issued within the last 3 months).
• If you are a new student coming to Hong Kong, you must present both the original and a copy of your one-way permit/visa identity card/passport/re-entry permit.
• The original and duplicate copies of the birth certificate of your siblings who are enrolled in our school, as well as a duplicate of the enrollment record page on the first page of their student handbook for this academic year (already stamped by the school).
• Parents or siblings who graduated from our school are requested to provide both the original and duplicate copies of their graduation certificates and identity documents (Schools where they graduated: Baptist Taisan Chen Yuanxi Primary School, Baptist Chen Yuanxi Primary School Afternoon School, Taisan Chamber of Commerce Huang Dadao Memorial School).
• The original and copies of the baptismal certificate or documentation proving the Christian faith of the parent(s) or the applicant child
• Application Form for Primary One Admission [i.e., the copy for parents issued by the Education Bureau (on yellow paper) or the completion of the “Smart Plus” application form]
• The original and duplicate copies of the student’s Hong Kong birth certificate
• If not born in Hong Kong, it is necessary to present the original and a copy of the birth certificate from another place, as well as the identity document proving residence in Hong Kong.
• Copies of identification documents for the student’s parents (both parents or guardian)
• The "Primary 1 Student Registration Form" that has been completed and attached with photos can be obtained from the School Administration Office or downloaded from the school website.
• I have already attached a return envelope [230mm x 100mm], which costs $2.2 in postage.
• I have already purchased a return mailing envelope [210mm x 297mm (A4 size)] that requires the use of stamps costing $7.9 each.

3. My elder siblings graduated from this school.
4. Parent or applicant: The child should have a Christian faith.
